Description
Under the direction of the Lake of the Woods District Hospital Program manager responsible for the SUD Pathways or designate, SUD System Navigator assists with creating seamless transitions within and between hospital and community services. This position facilitates timely access to the full spectrum of substance use services that are aligned with a client’s goals and preferences. This includes creating individualized care plans and ensuring best practice approaches to assessment, treatment, navigation, and support for individuals and families experiencing substance use disorders.
This position acts as a key connection point for patients, offering rapid response to substance use concerns, safety needs, and harm reduction. The SUD System Navigator facilitates seamless transitions between service providers and supports timely access to the full spectrum of healthcare services. Aligned with the five pillars that include supportive environments, peer support, harm reduction, substance use healthcare, and care transitions and navigation; the goal of this role is to improve access, enhance care quality, and support compassionate, evidence-informed services for individuals affected by substance use within the community.

Qualifications
- Post-secondary education in social services and/or health care or an applicable field, applicants with other combinations of applicable experience and education may be considered
- Current certification or the willingness to obtain First Aid and CPR
- Current certification or the willingness to obtain Naloxone, BLS and CPI Experience working with high-risk populations, particularly those who struggle with mental health and addictions
- Strong interpersonal skills with the ability to collaborate well with clients, family members, professionals and agencies
- Ability to work independently and as part of a team
- Knowledge of community resources and the ability to strongly advocate and make referrals
- Knowledge of harm reduction frameworks with a demonstrated ability to support individuals with complex needs
- Effective communication, organizational and time management skills
- Demonstrated proficiency with computers including the use of Microsoft Office products and electronic medical records
- Understanding of data collection for the purposes of statistical reporting and quality improvement
- A valid Ontario Driver’s License, with a clean driver’s abstract and regular access to a personal vehicle is required
- Reliable attendance record
